Has anyone successfully used this with an opt-in form? I have gotten it to work with a submit button but it seems finicky about using input labels...any ideas?
Also, I had the same problem as others where the form would display html within a position and it turned out to be an interference with another module using JS.

yes, i am using with ajcom news letter but having trouble showing on only specified itemd ids...any help.

copy and paste the HTML code from ajacom newsltter component and you are good to go..
